Thank you for considering Fox Ridge Medical Associates for your health care needs. In order to enhance your experience, we would like to provide you with several forms for you to complete in advance to streamline your visit. Please print out these forms and bring them to your first appointment. Please arrive 15 minutes in advance of your scheduled appointment, as we make every effort to remain on schedule. Although we do not accept Medicaid or traditional HMO’s, we accept most health insurance plans, including PPOs and Medicare.
